** You must be a member of Youth Group in order to attend YG events. **

 

1.	Complete the following *forms:   – Registration Form (Word Doc / PDF Doc)   – Permission Form (Word Doc / PDF Doc)   – Health Form ( Word Doc / PDF Doc)   * Print forms directly from this site or pick-up a registration packet from the YG folder located in the church narthex.
2.	Two ways to pay:   – Prepaid member: $25 (covers all events)   – Drop-in Member: $5 per event   – There are additional fees for premium events, such as Cedar Point & Whirly Ball.
3.	Submit all three forms and check (for prepaid members) to Mrs. Barber, YG Coordinator.
4.	Please make check payable to:  "St. Peter’s Youth Group," marked, "Registration Fee.".
5.	Parent Volunteers:    – 9th-12th Grade: Parents are asked to sign-up to help with three YG events.   – Contact the Youth Group Coordinator for further information.
6.	Friends:  If you invite a friend to one of our events, simply have their parents complete and sign a ‘YG Permission Form.’  Give completed form to the designated parent leader prior to the event.  If they are attending the Spring Retreat, they must have a completed Health Form, as well.
